Use this form to request a duplicate (replacement) vehicle title by mail. The duplicate title fee is $20.  When using this form to request a duplicate title by mail, you must include a photocopy of your identification.

Use this form to show proof of a vehicle ownership transfer through a private sale.

Statement of Transaction {Sales Tax Form} Sale or Gift of Motor Vehicle, ATV, Vessel (Boat) or Snowmobile

Use this form to certify sale price for sales tax calculation when vehicle, vessel, etc. is transferred in a casual sale or as a gift.

Claim for Sales Tax Exemption – Title/Registration Motor Vehicle, ATV, Vessel (Boat) or Snowmobile

Use when you are claiming exemption from sales tax for any reason other than a gift (then use DTF-802) or credit for taxes paid to other states (then use DTF-804)

DTF-804

Claim for Credit of Sales Tax Paid to Another State (For Motor Vehicle, ATV, Vessel (Boat) or Snowmobile)

Used when a New York resident purchases a vehicle out-of-state, registers the vehicle in New York State and applies for credit for the sales tax paid in the other state.
